WebThe Community Children’s Nursing Team (CCNT) provides highly skilled nursing care for children aged 0-18 years old in Hackney and the City of London. This service enables children to be cared for in their home preventing hospital admission and/or facilitating early discharge home, thereby allowing them to enjoy a normal family life. WebScreening activity will involve contacting parents/ carers and, where appropriate, children and young people, to understand their current situation and support needs. Once the screening process and conversations with the family has been completed, I then signpost to services that are deemed most appropriate to meet the needs of the child/ family.
